Office for Disability Services Scholarship Application Form

Scholarship Applications will only be accepted during January 15th through April 30th.

You must meet and check all requirements below in order for your application to be considered. Make certain all areas are complete prior to submitting your application:
GPA of 2.5 or higher:   I have a cumulative Penn State GPA of 2.5 or higher and have been enrolled at any Penn State University campus for at least one semester (12 credits) to be eligible to apply for any ODS scholarship.

Registered with ODS:   I am registered with the disability services office at any Penn State campus and meet the criteria for having a disability under the Americans with Disabilities Act, Amendments Act, 2008 and the Rehabilitation Act, 1973. This information will be verified by the disability office.

FAFSA filed:   I have filed a current FAFSA (applicants will not be considered without a current FAFSA in the Penn State financial aid system).

Met Scholarship guidelines:  

Filed Application on Time:   I have completed the entire application and I am submitting this application between January 15th and April 30th.

IMPORTANT: You must contact your Disability Specialist and/or your campus DCL and ask them to provide a copy of your current Academic Adjustment letter to the Scholarship Coordinator at The Pennsylvania State University, Office for Disability Services, 116 Boucke Building, University Park, PA 16802.
