Students will now submit a Certification Request Form for EVERY semester they plan to use VA benefits. All sections must be completed and submitted AFTER registration.


  1. University Park Certification Request Form  (Not For World Campus Students)
    You will need to download the Certification Request Form to your local computer, fill in the appropriate information, and then SIGN the form using a Digital Signature ID. You will be provided with instructions on creating an ID once you click the Student Signature form field.

    Once the form is complete and signed with your Digital Signature ID, return the PDF file via email to the Office of Veterans Programs at  or print a hard copy of the completed form and mail to:
